A Novel Precision Approach to Overcome the “Addiction Pandemic” by Incorporating Genetic Addiction Risk Severity (GARS) and Dopamine Homeostasis Restoration

چکیده
This article describes a unique therapeutic precision intervention, formulation of enkephalinase inhibitors, enkephalin, and dopamine-releasing neuronutrients, to induce dopamine homeostasis for detoxification treatment individuals genetically predisposed developing reward deficiency syndrome (RDS). The formulations are based on the results addiction risk severity (GARS) test. Based both neurogenetic epigenetic evidence, test evaluates presence genes alleles. Existing evidence demonstrates that novel genetic testing system can successfully stratify potential opioid use disorder (OUD) related risks or before initiating analgesic therapy RDS people in recovery. In case disorders, long-term maintenance agonist treatments like methadone buprenorphine may create RDS, have been existence, but not recognized. will also assess benefit from medication-assisted with augmentation. methodology holds strong promise reducing burden addictive disorders individuals, their families, society as whole by guiding restoration homeostasisthrough anti-reward allostatic neuroadaptations. We are proposing a generalized approach based on the Reward Deficiency Syndrome (RDS) conceptualization called the Systematic Medical Approach to Reward Transformation (SMARTTM). This system consists of: early pre-disposition diagnosis (even in children) using the Genetic Addiction Risk Score (GARS) [1]; a validated RDS questionnaire [2]; urine drug testing during actual treatment that uses com...

The interaction of neurotransmitters and genes that control the release of dopamine is the Brain Reward Cascade (BRC). Variations within the BRC, whether genetic or epigenetic, may predispose individuals to addictive behaviors and altered pain tolerance. This discussion authored by a group of concerned scientists and clinicians examines the Genetic Addiction Risk Score (GARS), the first test to...

The endemic of legal opioid iatrogenic induced prescription drug abuse is of major world-wide concern. Understanding pain pathways and the role of dopaminergic tone in the neurophysiology of pain relief provides potential therapeutic solutions. A 2011 NIDA report indicated that approximately 8.7% of the entire US population above the age of 12 years has used a psychoactive drug within the past ...

The central feature of drug addiction is compulsive drug of a motor activity, such as sniffing or biting (Randrup use—loss of control over apparently voluntary acts of and Munkvad, 1967). Addicrepeatedly, some acute drug effects may diminish (“toltion is a chronic disorder, since even after treatment erance”), whi...

PROBLEM Substance use is highly prevalent in the United States, but little time in the curriculum is devoted to training internal medicine residents in addiction medicine. APPROACH In 2014, the authors developed and launched the Addiction Recovery Clinic (ARC) to address this educational gap while also providing outpatient clinical services to patients with substance use disorders. The ARC is...
